Social care and transport

For some people, getting older can mean getting out and about becomes more difficult. To support older people with mobility, or other health problems including early dementia, St Luke's offers a social care and transport service.

What does our social care service offer?

  • a programme of stimulating activities and outings
  • a warm, safe and comfortable environment
  • qualified and trained social care staff and volunteers
  • ongoing reviews to ensure we meet the changing needs of each person
  • respite for carers
  • a freshly cooked subsidised 3 course lunch
  • an escorted door-to-door transport service.
  • professional experience of caring for people with dementia and other age-related health problems.

Who can attend?

Our social care service is available to any older person living within the area of benefit who is also a member of St Luke's, and who would otherwise be unable to participate at the Centre.

In order to get the most out of coming to our Centre, we encourage our social care services users to take part in the activities and stimulation offered, without causing undue distress or disruption to themselves or others.

Finally, members need to be able to stand up from a seated position independently or with minimal assistance.

How to apply for the social care service

All enquiries should be directed to our Care Manager, Jem Wilson on 020 7549 8190 or email jwilson@slpt.org.uk. Self referrals are welcomed and referrals can also be made by social workers, GPs, family and friends.

We will arrange for members and carers to visit the Centre to see what we offer and whether this meets their needs. A senior member of our care team will then assess prospective social care member to confirm that the services are appropriate and suitable.

Care plans are negotiated for each member as part of our referral and assessment process. They are reviewed regularly so we can be certain the service meets the changing needs of both the member and their carers.

Transport

St Luke's provides a daily transport service to ensure older people with mobility or other health problems can still enjoy the Centre and feel part of their community.

We own a fully accessible mini bus and employ a MIDAS qualified and experienced mini bus driver/carer. Our transport service is a door-to-door escorted service and available to all social care members. In addition, all members can register themselves on the 'occasional users' transport list and take lifts to or from the Centre if the weather is poor or they are feeling unwell.

St Luke's Heart & Stroke Club

The St Luke's Heart and Stroke Club is a specific care service delivered by staff and volunteers from Manor Gardens Heart & Stroke Club every Wednesday from 10.30am-3.30pm. Club members live in South Islington and have either had a stroke or are coping with heart problems. This friendly and sociable club offers a wide range of activities including exercise, massage, advice, advocacy and information.
